Activating home-delivered meal benefits for those who are food insecure and nutritionally at-risk in New York is easy, just follow the steps below.

1) Identify

Identify clients at risk for food or nutrition insecurity during the initial assessment/reassessment/change of status to trigger a home delivered meal benefit to support a plan of care.

2) Authorize and Process

MLTC and State of New York

Complete your agency’s required information and demographics letter or complete Mom’s Meals Authorization Form and submit using one of the following options:

  • email to: intake@momsmeals.com
  • Fax to: 515.266.6120
  • call 1-866-716-3257, option 1
  • Your specific agency’s portal

You should receive confirmation email within 48 hours (sent to the email on file). If you do not receive confirmation or need additional support, please email intakesupport@momsmeals.com OR contact Colleen Miller at colleen.miller@momsmeals.com

3) Delivery

A cooler of fully-prepared, refrigerated meals will be delivered to the client's home - ready to heat, eat and enjoy within minutes.
