Expert to Highlight Role of Nutrition in Improving Post-Acute Patient Health

Catherine Macpherson, registered dietitian nutritionist and Chief Nutrition Officer at Mom’s Meals NourishCare, the nation’s leading provider of home-delivered meals for older adults, people recently hospitalized, and people with chronic conditions, will be co-hosting a live webinar for the Academy of Nutrition and Dietetics on September 12, 2018, in conjunction with September’s designation as Malnutrition Awareness Month.  Macpherson will present Transitioning from Hospital to Community: Malnutrition Past, Present and Future.  

“Programs that improve nutrition after an acute event are critical for healing and recovery,” said Macpherson. “When people are moving from one point of care to another, for instance hospital to home, there is a change in the care team as well as changes in condition and health status. A successful transition requires evaluating needs and planning for support of the individual. This should include nutrition status, particularly as malnourished patients are twice as likely to be readmitted to the hospital.”

The hour-long webinar will focus on care transitions and highlight nutrition as an essential  component of post-care programs to address patient health. Studies now show malnutrition plays a detrimental role in post-acute community and home settings and increases patient utilization of health services driving up costs. Additionally, studies show that addressing patient nutrition through post-discharge prepared and specialized meals can positively impact health outcomes and reduce hospital readmissions.

Macpherson will also highlight how registered dietitian nutritionists can be part of the solution by becoming involved in post-acute models of care through their work with health systems and hospitals, community organizations, area agencies on aging, and health plans to introduce innovative nutrition programs and models to benefit care transitions.

The webinar will be co-presented with Jacob T. Mey, Postdoctoral Research Fellow, Pennington Biomedical Research Center, Integrated Physiology and Molecular Metabolism, and facilitated through The Academy of Nutrition and Dietetics, the world’s largest organization of food and nutrition professionals. The Academy is committed to improving the public’s health and advancing the profession of dietetics through research, education and advocacy.
