Authorizations & Referrals

Activating home-delivered meal benefits for those who are food insecure and nutritionally at-risk in Pennsylvania is easy, just follow the steps below.

1) Identify

Identify clients at risk for food or nutrition insecurity during the initial assessment/reassessment/change of status to trigger a home delivered meal benefit to support a plan of care.

2) Authorize and Process

For Community Health Choices:

  • For those with access to a portal, generate referral/authorization through the organization’s designated portal system. Authorizations are received through the portal within 72 hours.
  • For those unable to submit via a portal, download a referral form (not required if submitting authorization through portal unless additional information is required) and submit using one of the following options:

Mom's Meals Intake processes the home delivered meal order and sends confirmation of start of service to the referring Service Coordinator.

Coding: W1760 – Standard meals | W1764 – Pureed meals.

LIFE/PACE/AAA: OAA, SSA Block Grant, other funding sources are available. Benefits are determined by the organization and require an authorization form either provided by the organization or Mom’s Meals.

3) Delivery

A cooler of fully-prepared, refrigerated meals will be delivered to the client's home - ready to heat, eat and enjoy within minutes.
