Authorizations & Referrals
Activating home-delivered meal benefits for those who are food insecure and nutritionally at-risk in Texas is easy, just follow the steps below.
1) Identify
Identify clients at risk for food or nutrition insecurity during the initial assessment/reassessment/change of status to trigger a home delivered meal benefit to support a plan of care.
2) Authorize and Process
- For Molina, Amerigroup, Superior, Cigna, UHC and possibly others that have a footprint in the state of Texas, use your own portal to submit authorizations to Mom’s Meals.
- For those who don’t have access to a portal or if your portal is down, submit using one of the following options:

After the authorization is received, Mom’s Meals will verify and confirm eligibility against the MCO portal. Mom’s Meals will reach out after verification has been completed and begin processing the order.
3) Delivery
A cooler of fully-prepared, refrigerated meals will be delivered to the client's home - ready to heat, eat and enjoy within minutes.
