- Do you have chronic conditions, gestational diabetes or other high-risk pregnancy conditions, chronic or disabling mental or behavioral health disorders?
- Are you a patient discharging from the hospital or skilled nursing facility or are at high risk of hospitalization or nursing home placement?
- Do you have care needs that require multiple doctors?
