Activating home-delivered meal benefits for those who are food insecure and nutritionally at-risk in Florida is easy, just follow the steps below.
Identify clients at risk for food or nutrition insecurity during the initial assessment/reassessment/change of status to trigger a home delivered meal benefit to support a plan of care.
2) Authorize and Process
- Case manager or utilization team enters authorized meals into organization's designated portal system.
- The portal generates an authorization which is forwarded by the case manager or utilization team to Mom’s Meals and submit it in the portal and submit using one of the following options:
- Mom’s Meals intake team processes authorization and sends confirmation of start date to the case manager or utilization team via email.
A cooler of fully-prepared, refrigerated meals will be delivered to the client's home - ready to heat, eat and enjoy within minutes.
