Activating home-delivered meal benefits for those who are food insecure and nutritionally at-risk in Iowa is easy, just follow the steps below.
1) Identify
Identify clients at risk for food or nutrition insecurity during the initial assessment/reassessment/change of status to trigger a home delivered meal benefit to support a plan of care.
2) Authorize and Process
MCO WAIVER: AIDS/HIV, ELDERLY, HEALTH & DISABILITY
- Individual MCO ISP/authorization submitted to MCO internal care coordinator or to Mom’s Meals via email at intake@momsmeals.com or fax: 515-266-6120.
- 14 meals every week (2 meals per day) or up to 62 meals a month
- Two codes are required
- Morning code: S5170 U
- Noon code: S5170 UG
- Evening code: S5170 UH
PACE/AAA: OAA, SSA Block Grant, Other funding
Meals authorized on a service authorization form (SAF) listing number of meals authorized sent via mail at intake@momsmeals.com or fax: 515-266-6120.
- Benefit determined by the organization (minimum 10 meals every two weeks)
- No code required
Mom’s Meals intake team processes authorization and member receives meals within 5 business days. Meals will continue to be delivered until authorization end date or terminated by MCO/client or AAA/Pace or authorizing agency.
3) Delivery
A cooler of fully-prepared, refrigerated meals will be delivered to the client's home - ready to heat, eat and enjoy within minutes.
