Authorizations & Referrals
Activating home-delivered meal benefits for those who are food insecure and nutritionally at-risk in Minnesota is easy, just follow the steps below.
Identify clients at risk for food or nutrition insecurity during the initial assessment/reassessment/change of status to trigger a home-delivered meal benefit to support a plan of care.
Complete your SAF and send it to Mom's Meals using one of the following options:
- email: intake@momsmeals.com
- fax: 515.266.6120 (option 1)
- Regular mail
Once the form is received, Mom’s Meals will verify and confirm eligibility against the state or MCO portal where applicable. The meal benefit is 1 meal/day – 14 meals delivered every two weeks. Those that have an Exception To Benefits which would allow for 1 additional meal/day.

A cooler of fully-prepared, refrigerated meals will be delivered to the client's home - ready to heat, eat and enjoy within minutes.
