Activating home-delivered meal benefits for those who are food insecure and nutritionally at-risk in Ohio is easy, just follow the steps below.

1) Identify

Identify clients at risk for food or nutrition insecurity during the initial assessment/reassessment/change of status to trigger a home delivered meal benefit to support a plan of care.

2) Authorize and Process

PASSPORT/MYCARE/OHC:

Complete your organization’s referral or authorization form and submit using one of the following options:

Mom’s Meals Intake will process the home delivered meal order and will send confirmation of start of service to the referring Case Manager

DODD – Department of Developmental Disabilities

Please note: Due to a change to Ohio Administrative Code 5123-2-08 (G)(8), as of 3/7/2022, all Mom’s Meals clients covered in the counties listed below under an AMN OR FMN (level 1) waiver will be moved to weekly meal delivery. The minimum for these deliveries is:

  • 5 meals per week for existing clients
  • 7 meals per week for new authorizations

Counties served: Adams, Ashtabula, Butler, Carroll, Champaign, Clark, Clinton, Columbiana, Cuyahoga, Defiance, Delaware, Fairfield, Fayette, Franklin, Geauga, Greene County, Hamilton County, Henry, Lake, Licking, Lorain, Lucas, Madison, Mahoning, Marion, Medina County, Miami County, Montgomery, Ottawa, Pickaway, Portage, Sandusky, Scioto, Seneca, Stark County, Summit County, Trumbull County, Tuscarawas, Union, Warren, Williams, Wood.

Complete an ISP for qualifying clients and submit using one of the following options:

The ISP should include the following:

  • Member’s address
  • Member’s phone number
  • Member’s DOB
  • Authorizing agency and SSA information
  • Desired beginning and end dates of service
  • Number of meals client is approved for per week
  • Menu information or any special requests
  • Billing and member delivery information

Mom’s Meals will review and process the ISP, sign it, and return it to be entered into the PAWS PORTAL. Once the PAWS PORTAL approves the budget, the member can start receiving meals immediately.

3) Delivery

A cooler of fully-prepared, refrigerated meals will be delivered to the client's home - ready to heat, eat and enjoy within minutes.
