Mom’s Meals® dietitians on our commercial team serve as the clinical backbone of our partnerships. They help health plans, health systems, state governments and other organizations build a program of cost-effective and proven food as medicine solutions for their members of all ages.
What is food as medicine?
While there is no single definition of the food as medicine concept, recognition has shifted from nutrition as only the food we eat, to a clinical tool to prevent and manage chronic diseases, as well as having the ability to build, maintain and restore health.
Mom’s Meals, the most comprehensive provider of food as medicine solutions nationwide, views food as medicine as a stabilizing force empowering individuals to live healthier lives. To maximize the effectiveness of food as medicine, it must be practical, scalable and integrated into the health care system by working with health care payers, providers and community partners to deliver tailored nutrition services and access to evidence-based solutions to help support independent living, better manage chronic diseases, improve health outcomes and reduce the total cost of care.
Mom’s Meals dietitians support partnerships for meaningful programs
The registered dietitians (RDs) and registered dietitian nutritionists (RDNs) on our commercial team provide dedicated and tailored account management for our partners. It is this hands-on approach combined with clinical expertise that helps partners determine the right food as medicine solutions to serve the diverse needs of their Medicaid, Medicare Advantage and commercial populations including those with chronic conditions, older Americans, those recovering from a hospital stay, expecting and new mothers, as well as rural and underserved populations.
“We have dietitians involved in business development and account management," shares Catherine Macpherson, MS, RDN and senior vice president and chief nutrition officer at Mom’s Meals. "They are also very knowledgeable about the health care system and the technical setup of how services are paid for. This means our partners can rely on our breadth of knowledge and experience in the industry.”
Mom’s Meals Fresh Foundations Nutrition Care PlatformTM
Managing nutrition needs across diverse populations requires more than a single program or intervention. The Fresh Foundations Nutrition Care Platform (Fresh Foundations) from Mom's Meals provides a clinically governed system for deploying personalized nutrition care that supports better health outcomes while helping health plans and state programs scale food as medicine strategies with consistency and oversight.
By combining Mom's Meals nutrition capabilities with clinically guided progression pathways, the platform helps organizations deliver nutrition care that is structured, measurable and adaptable to evolving member needs.
Through Fresh Foundations, health plans and state agencies can help members:
- Stabilize nutrition and better manage chronic and other health conditions
- Reduce avoidable utilization and support recovery at home
- Support clinically guided progression to independence
- Scale food as medicine strategies across populations
Clinical translation
Using data, insights and clinical observations, the commercial team of dietitians provide partners with valuable information that helps make decision-making easier and create actionable, results-oriented solutions. A few examples of this clinical translation and support include:
- Data-informed decision making — They help their partners understand how clinical data can inform important business metrics. Whether it is determining what is needed to achieve Star Ratings, HEDIS measures or member satisfaction, RDs and RDNs can help answer those questions with evidence-based data.
- Impact reporting — Program-specific reporting and explanation helps each organization track, for example, not just meal volume and delivery speed, but also health improvements such as A1C levels, medication adherence, reduction in readmissions and emergency room visits of the members served.
- Case study development and utilization — Mom’s Meals has taken part in a variety of clinical pilots and programs and co-developed case studies that have also appeared in academic journals that prove the effectiveness of using tailored nutrition to improve health outcomes. Our dietitians use this information and leverage a library of peer-reviewed evidence to help partners develop evidenced-based programs and benefits that are right for their organization.
Navigating the health care ecosystem
Beyond nutrition, our RDNs possess deep technical knowledge of the health care system, including billing processes, CMS requirements, and digital referral integration. They work to find efficiencies, bringing additional value to partnerships.
Advocacy for broader access
The impact of our dietitians also extends to state and federal policy. Working alongside our government affairs team, they serve as subject matter experts who advocate for broader reimbursement and coverage for using food as medicine as part of health care.
