Meet our nutrition counseling and education RDNs

Using food as medicine can improve health outcomes and lower total cost of care. But for many people — especially those going home from the hospital, managing chronic illnesses, or navigating prenatal and postpartum care — getting quality nutrition and making healthy lifestyle changes can seem challenging. The expert, science-based guidance and support from the Mom’s Meals® nutrition counseling and education team of registered dietitians (RDs) and registered dietitian nutritionists (RDNs) is an effective solution.

Personalized counseling and education

Registered dietitians or RDNs are experts in the disciplines of food and nutrition. They translate the complex science of nutrition into healthy, real-world solutions for clients. Their credentials have the same meaning, and the “N” is an option to RDs who want to emphasize the nutrition aspect of their credential (going forward referring to both as RDN).

Getting started

If a health plan requires a dietitian assessment as part of the authorization process to receive nutrition support, such as our medically tailored home-delivered meals, that plan member will be contacted by one of our RDNs. The assessment will take about 15 minutes to complete.

  • The dietitian will make sure they are speaking to the right person and verify the address for meal delivery that will be used if approved by the member’s health plan to receive medically tailored meals
  • Questions will be asked about chronic health conditions, recent hospitalizations, current eating habits, mobility concerns and other health-related topics
  • The RDN will inform the member of next steps for their nutrition program

“Sometimes a hurdle in member engagement is a misunderstanding of what an RD or RDN actually does,” says Aimee Mason, MS, RDN, who manages the team of RDNs. “On our initial call, we get information that will help us tailor and set goals for any subsequent follow up. This is an opportunity to tailor, educate, and build on the good choices members may already be making.”

Count on ongoing support

The team of RDNs use tools and clinical data, such as A1C, blood pressure and food insecurity screenings to tailor their approach. This personalized touch has led to a high engagement rate, with members frequently calling back specifically to speak with their dietitian.

Our nutrition counseling and education program is grounded in positive reinforcement and attainable goals. An RDN is there to meet the member where they are and find meals and nutrition options that will work for that unique individual. Whether a member is struggling with mobility issues that prevent them from standing at a stove or dealing with a new diagnosis like diabetes that feels overwhelming, an RDN acts as a clinical partner to help navigate those barriers and provide an action plan.

Nutrition counseling and education is especially helpful in improving outcomes for individuals with nutrition-sensitive chronic health conditions including:

  • Heart disease
  • Chronic kidney disease

  • Dysphagia
  • High-risk perinatal conditions
  • Mental or behavioral health disorders

For one person, it might be lowering an A1C, so they don't have to start a new medication. It could be helping an expecting mother have the nutrition and support services she needs for a healthy pregnancy and delivery. For another, it might mean having enough nutrition security to get through the month without financial stress.”
