As part of the collaboration, an initial pilot is underway with a value-based care organization in Western Massachusetts to support up to 500 Medicare Advantage members managing conditions including heart failure, chronic kidney disease and cancer. Eligible members may receive home-delivered, medically tailored meals as part of a pilot designed to assess feasibility, engagement and the role nutrition can play in more proactive care.
Disease-associated malnutrition is estimated to cost the U.S. healthcare system $156.7 billion annually, contributing to avoidable hospitalizations, longer hospital stays and poorer outcomes for individuals managing serious chronic conditions, underscoring the need for scalable nutrition interventions that can reach people where they live.
