How can they get home-delivered meals?
Your patient or client may be eligible for FREE home- delivered meals from Mom’s Meals® through their Medi-Cal health plan as part of the California Community Supports Program if they have one or more nutrition-sensitive chronic health conditions (e.g., diabetes, heart disease, cancer, obesity, COPD, stroke, mental health disorders, high-risk pregnancy, and more).
To refer, you'll need the patient’s health plan name and member ID number to complete a short form.
*Eligibility criteria may vary by health plan. Some plans may also require a nutrition assessment and ongoing nutritional counseling.
