How Do I Get Home-Delivered Meals?

You may be eligible for FREE home-delivered meals from Mom's Meals^® through your Medi-Cal health plan as part of the California Community Supports Program.

You may qualify if you have one or more nutrition-sensitive chronic health conditions, such as diabetes, heart disease, cancer, obesity, COPD, stroke, mental health conditions, high-risk pregnancy, and others.?

To get started, fill out a short form. Have your health plan name and member ID ready. The form includes a full list of qualifying health conditions.
