How to Qualify
Here are some of the most common ways you may be eligible for home-delivered meals from Mom's Meals.
Can you or someone you support answer "yes" to all of the following questions?
- Enrolled in a Louisiana Medicaid health plan?
- Pregnant or postpartum?
- Identify as part of a BIPOC community, have a high-risk pregnancy condition or are experiencing food insecurity?
If so, you may be eligible for home delivered meals through the Louisiana's Maternal Health benefit. To get started, reach out to your care team for a referral.
We offer home-delivered to eligible clients, including:
- Louisiana Community Choice Waiver members
- Medicaid Waiver members recovering after a hospital stay, managing a chronic condition or are pregnant or postpartum
- OAA program recipients
- PACE program participants
- People who require assistance with grocery shopping and/or meal preparation
Contact your case manager or service coordinator to find out if you qualify.
If your MA health plan includes home-delivered meals as a covered benefit, you may qualify if you:
- Have one or more chronic health conditions
- Are a patient discharging from a hospital or skilled nursing facility or are at high risk of hospitalization
Check your health plan coverage manual to confirm if medically tailored meals are included and if you're eligible to receive them.
If you or a loved one do not qualify for one of the above programs, you can purchase meals direct from Mom’s Meals. Our meals are priced at $9.49 or less per meal. You can count on:
- Service that starts quickly
- Meals to help care for a loved one from near or far
- Peace of mind knowing you or your loved one are eating well
What is the Maternal Health benefit?
Louisiana's Maternal Health benefit helps support a healthy pregnancy and recovery after birth.
Who is eligible?
You may qualify if you:
- Are enrolled in a Louisiana Medicaid health plan
- Are pregnant or postpartum
- Meet at least one of the following criteria:
- Identify as part of a BIPOC community
- Have a high-risk pregnancy condition, including:
- Hypertension or gestational hypertension
- Diabetes or gestational diabetes
- History of preterm labor or preterm birth
- Experiencing food insecurity
What's included?
If eligible, you may receive:
- Up to 8 months of nutrition support
- 20 medically tailored meals delivered to your home every other week at no cost
