How to Qualify
Here are some of the most common ways you may be eligible for home-delivered meals from Mom's Meals.
Can you or someone you support answer "yes" to all of the following questions?
- Enrolled in a Medicaid Plan?
- Living with a health condition such as diabetes, heart disease, COPD, kidney disease or cancer with malnutrition?
- Experiencing food insecurity of challenges preparing meals due to physical, mental or cognitive limitations?

We offer home-delivered and site-delivered meals to eligible clients, including:
- Medicaid Waiver members (e.g. MI Choice Waiver, MI Health Link)
- OAA program recipients
- PACE program participants
- New and expecting mothers
- People who require assistance with grocery shopping and/or meal preparation
Contact your case manager or service coordinator to find out if you qualify.
If your MA health plan includes home-delivered meals as a covered benefit, you may qualify if you:
- Have one or more chronic health conditions
- Are a patient discharging from a hospital or skilled nursing facility or are at high risk of hospitalization
Check your health plan coverage manual to confirm if medically tailored meals are included and if you're eligible to receive them.
If you or a loved one do not qualify for one of the above programs, you can purchase meals direct from Mom’s Meals. Our meals are priced at $9.49 or less per meal. You can count on:

What are Michigan’s Health-Related Social Needs (HRSN) services?
The HRSN services help eligible Michigan Medicaid plan members access nutrition focused support that promote better health outcomes, including:
- Medically Tailored Home Delivered Meals
- Healthy Home Delivered Meals
- Healthy Food Packs
- Produce Prescriptions


Who is eligible for meals?
To qualify, individuals must meet all of the following:
- Be enrolled in a participating Medicaid health plan
- Screened and identified as food insecure

Have a clinical risk factor, such as the follow nutrition-sensitive conditions:
- Diabetes
- Congestive Heart Failure (CHF)
- Cardiovascular Disorder
- Hypertension
- HIV
- Cancer
- Sickle Cell Disease
- Renal/Kidney Disease
- Malnutrition
- Pregnancy-related complications (e.g., gestational diabetes, preeclampsia, dehydration)
- Recent hospital or skilled nursing facility discharge (within 90 days)
- Former foster care youth with chronic illness
- Children with high blood lead levels, obesity, eligible for Children’s Special Health Care Services (CSHCS) or Persons with Special Health Care Needs (PSHCN)

- Food insecurity
- Limited ability to shop or prepare meals due to physical, mental, or cognitive challenges
