How to Qualify

Here are some of the most common ways you may be eligible for home-delivered meals from Mom's Meals.

Can you or someone you support answer "yes" to all of the following questions?

  • Enrolled in a New Mexico Medicaid health plan?
  • Pregnant and diagnosed with Type 1, Type 2, or gestational diabetes?

If so, you may be eligible for home delivered meals through the Food Is Medicine (FIM) benefit. To get started, complete a short referral form and send it to the your health plan:

Can you or someone you support answer "yes" to all of the following questions?

  • Enrolled in a New Mexico Medicaid health plan?
  • Meet Level 2 Nursing Facility Level of Care (NFLOC) requirements?
  • Experiencing food insecurity?
  • Have medical or functional needs that can be supported through nutrition?

If so, you may be eligible for home-delivered meals through New Mexico's Community Benefit. Ask your case manager or contact your health plan using the number on the back of your insurance card to see if you qualify.

We offer home-delivered and site-delivered meals to eligible clients, including:

  • Medicaid Waiver members
  • OAA program recipients
  • PACE program participants
  • New and expecting mothers
  • People who require assistance with grocery shopping and/or meal preparation

Contact your case manager or service coordinator to find out if you qualify.

If your MA health plan includes home-delivered meals as a covered benefit, you may qualify if you:

  • Have one or more chronic health conditions
  • Are a patient discharging from a hospital or skilled nursing facility or are at high risk of hospitalization

Check your health plan coverage manual to confirm if medically tailored meals are included and if you're eligible to receive them.

If you or a loved one do not qualify for one of the above programs, you can purchase meals direct from Mom’s Meals. Our meals are priced at $9.49 or less per meal. You can count on:

  • Service that starts quickly
  • Meals to help care for a loved one from near or far
  • Peace of mind knowing you or your loved one are eating well

What is the Maternal Food is Medicine (FIM) Benefit?

New Mexico's Maternal Food is Medicine (FIM) benefit helps support a healthy pregnancy and recovery after birth. Eligible pregnant Medicaid members can receive nutrition support to help manage diabetes and support mom and baby.

Who is eligible?

You may qualify if you:

  • Are enrolled in a New Mexico Medicaid health plan
  • Are pregnant
  • Have Type 1 diabetes, Type 2 diabetes, or gestational diabetes

What's included?

If eligible, you may receive:

  • Up to 2 medically tailored meals delivered to your home each day, or a grocery box
  • Support for up to 11 months during pregnancy plus 2 months postpartum

What is the Community Benefit?

New Mexico's Community Benefit helps Medicaid members with long-term care needs get the nutrition support they need to stay healthy and independent at home.

Who is eligible?

You may qualify if you:

  • Are enrolled in a New Mexico Medicaid health plan
  • Meet Level 2 Nursing Facility Level of Care (NFLOC) requirements
  • Are experiencing food insecurity
  • Have medical or functional needs that can be supported through nutrition

You may still qualify even if you already receive meals through a senior center, the Older Americans Act (OAA), or another food assistance program.

What's included?

If eligible, you may receive:

  • Up to 2 medically tailored meals delivered to your home each day
  • Meals designed to support your health condition and care plan
  • Nutrition support to help meet dietary needs and stay on track with treatment plans
