Answering “yes” to any of the questions below may mean you’re eligible for home-delivered meals from Mom’s Meals through government assistance or a health plan benefit!
Managed Long-Term Services and Supports (MLTSS) Eligibility
Can you or someone you support answer "yes" to any of these questions?
- Age 65 or older?
- Living with a disability?
- Require assistance with grocery shopping and/or preparing meals?
- Enrolled in a Medicaid or a Medicare Advantage plan that includes a meal benefit?
Participants in the MLTSS program should contact their case manager or health plan to see if they are eligible to receive a home-delivered meals benefit.
Health-Related Social Needs (HRSN) Waiver Eligibility
Can you or someone you support answer "yes" to all the following questions?
- Enrolled in Medicaid Managed Care?
- Screened and identified as food insecure
- Belong to at least one Enhanced Population?
- Have at least one chronic condition?

A patient or client can purchase home-delivered meals directly from Mom's Meals. Our meals are priced at $9.59 or less per meal. That's an affordable price that fits almost any budget. They also receive discounts when they choose our Subscribe & Save option.

Health-Related Social Needs (HRSN) Waiver Eligibility for Home-Delivered Meals
The HRSN services help eligible New York Medicaid plan members access non-medical supports that promote better health outcomes, including:
- Food and nutrition (e.g., medically tailored home-delivered meals, food prescriptions, pantry stocking)
- Housing
- Transportation
- Social care management
Nutrition Counseling & Education
Some plans may require a nutrition assessment and ongoing nutritional counseling.
