How to Qualify for Home-Delivered Meals in New York
Here are some of the most common ways you may be eligible for home-delivered meals from Mom's Meals through government assistance or a health plan benefit.
Medicaid Members with Health Related Social Needs (HRSN)
Can you or someone you support answer "yes" to all of the following questions?
- Enrolled in Medicaid Managed Care?
- Screened and identified as food insecure?
- Belong to at least one Enhanced Population?
- Have at least one chronic condition?

Medicaid Members, Older Americans Act (OAA) Recipients, and PACE Program Participants
We offer home-delivered and site-delivered meals to eligible clients, including:
- Medicaid Waiver members
- OAA program recipients
- PACE program participants
- New and expecting mothers
- People who require assistance with grocery shopping and/or meal preparation

Medicare Advantage (MA) Health Plan Members
If your MA health plan includes home-delivered meals as a covered benefit, you may qualify if you:
- Have one or more chronic health conditions
- Are a patient discharging from a hospital or skilled nursing facility or are at high risk of hospitalization
Check your health plan coverage manual to confirm if medically tailored meals are included and if you're eligible to receive them.

What are Health Related Social Need (HRSN) services?
The HRSN services help eligible New York Medicaid plan members access non-medical supports that promote better health outcomes, including:
- Food and nutrition (e.g., medically tailored home-delivered meals, food prescriptions, pantry stocking)
- Housing
- Transportation
- Social care management


Who is eligible for meals?
To qualify, individuals must meet all of the following:
- Enrolled in Medicaid Managed Care
- Screened and identified as food insecure

Belong to at least one Enhanced Population, such as:
- High Medicaid utilizers or Health Home enrollees
- Individuals with SUD, SMI, or I/DD
- Pregnant or postpartum persons
- Recently incarcerated (within 90 days) with a chronic condition
- Youth in foster care or juvenile justice
- Children at high-risk or with chronic conditions

- Congestive Heart Failure or Hypertension
- COPD or Asthma
