How to Qualify

Here are some of the most common ways you may be eligible for home-delivered meals from Mom's Meals.

If you’ve paid into the WA Cares Fund and need help with three or more daily activities, you may be eligible for benefits that can be used for services such as home-delivered meals.

Can you or someone you support answer "yes" to all of the following questions?

  • Enrolled in Apple Health Medicaid plan?
  • Screened and identified as food insecure?
  • At high risk of hospitalization or institutionalization, recently discharged from the hospital, or pregnant/postpartum?
  • Have at least one chronic condition?

If you believe you qualify, reach out to your Accountable Communities for Health (ACH) to make a referral.

We offer home-delivered and site-delivered meals to eligible clients, including:

  • Medicaid Waiver members, such as:
    • Community Options Program Entry System (COPES)
    • Medicaid Alternative Care (MAC)
    • Tailored Supports for Older Adults (TSOA)
    • New Freedom
  • OAA program recipients
  • PACE program participants
  • New and expecting mothers
  • People who require assistance with grocery shopping and/or meal preparation

Contact your case manager or service coordinator to find out if you qualify.

If your MA health plan includes home-delivered meals as a covered benefit, you may qualify if you:

  • Have one or more chronic health conditions
  • Are a patient discharging from a hospital or skilled nursing facility or are at high risk of hospitalization

Check your health plan coverage manual to confirm if medically tailored meals are included and if you're eligible to receive them.

If you or a loved one do not qualify for one of the above programs, you can purchase meals direct from Mom’s Meals. Our meals are priced at $9.49 or less per meal. You can count on:

What is the WA Cares Fund?

WA Cares Fund is Washington State's long-term care program. Eligible individuals who worked in Washington and contributed to the WA Cares Fund may use benefits for services that help them remain safe and indenpdent at home. Services include:

  • Home-delivered meals – Nutritious meals delivered to your home
  • In-home care – Help with personal care and household tasks
  • Residential care – Support in adult family homes, assisted living, or nursing homes
  • Home modifications – Safety updates like ramps and grab bars
  • Transportation – Rides to doctor visits and errands
  • Assistive equipment – Devices like bath chairs and emergency alert systems
  • Care supplies – Hygiene aids and incontinence products
  • Caregiver support – Training, respite care, and help with care planning

Who is eligible for WA Cares-funded meals?

To qualify, individuals must meet both requirements:

1. Worked in Washington and paid into WA Cares
The person receiving care must have worked in Washington and contributed to the WA Cares Fund through payroll deductions.

They may qualify if they:

  • Contributed for 10 or more years (full benefit)
  • Contributed for 3 of the past 6 years (full benefit)
  • Were born before January 1, 1968 and contributed for at least 1 year (partial benefit)

Benefits may be available up to $36,500.

2. Have qualifying care needs
They must need help with at least three activities of daily living for at least 90 days or more, such as:

  • Eating
  • Bathing or toileting
  • Mobility or transferring
  • Dressing or personal care

If you meet these requirements, you must complete your application and assessment with the state.
