Our RDNs can help members transition from a structured program to nutrition self-management. RDNs conduct motivational interviewing to customize an educational approach for each member by:

  • Actively listening to members
  • Meeting members where they’re at
  • Matching education to health conditions, needs and preferences

Members practice on their learned skills, such as:

  • Reading nutrition labels
  • Simple meal prep
  • Managing portion sizes

RDNs and members will strive for the goal of: Improving important condition metrics, including blood pressure, blood sugar readings and A1c levels.

Why a Mom’s Meals Dietitian May Contact You

If your health plan requires a dietitian assessment as part of the authorization process to receive Mom’s Meals home-delivered meals, you will be contacted by one of our dietitians.

  • The dietitian will make sure they are speaking to the right person and verify the address for meal delivery that will be used if approved, by your health plan, to receive medically tailored meals
  • Questions will be asked about chronic health conditions, recent hospitalizations, current eating habits, mobility concerns and other health-related topics
  • The assessment will take about 15 minutes to complete
  • The dietitian will inform you of next steps for your nutrition program
