Mom’s Meals®, a leading provider of medically tailored, home-delivered meals, and USAging, the national association representing and supporting the national network of Area Agencies on Aging, are seeking nominations for the 2024 Community Champion Award. This award underscores the value of the case manager’s role in supporting whole health in their communities and promotes awareness of the importance of nutrition in overall health — particularly to reduce the impact of social determinants, to support individuals who want to age at home, and to support the management of chronic conditions.
“Case managers play a critical role in helping those who face many social determinants of health,” said John Phillips, vice president of national program development for Mom’s Meals. “When it comes to food insecurity, they work tirelessly to ensure access to healthier foods to help their clients remain at home, stay out of the hospital and better manage chronic conditions. We are proud to continue our partnership with USAging to recognize case managers for their outstanding work in their local communities.”

Nominations are currently being accepted for this year’s award. The 2024 Community Champion will be announced at the USAging Annual Conference July 8-11 in Tampa, Florida. Nominees do not have to be contracted with Mom’s Meals to be eligible; however, they must be from a USAging member agency. The last day for nominations is Friday, April 19.
