Nominations open for 2026 Community Champion Award

Mom’s Meals®, a leading food as medicine solutions provider, and USAging, the national association representing and supporting the national network of Area Agencies on Aging, are seeking nominations for the 2026 Community Champion Award. 

The annual award honors case managers who demonstrate exceptional commitment to improving health and well-being by connecting individuals with nutrition and other essential services that help them live healthier lives at home. It also highlights the important role nutrition plays in supporting healthy aging and managing chronic conditions. 

“Case managers are often the link between individuals and the services that make it possible to live safely and independently at home,” said John Phillips, vice president of national program development for Mom’s Meals. “By helping connect people with nutrition and other critical supports, they play an important role in improving health and quality of life. Through our partnership with USAging, we are honored to recognize the dedication and impact case managers have in communities across the country.” 

“Case managers are on the front lines helping older adults connect to and navigate the services that support their health, independence and quality of life,” said Sandy Markwood, USAging chief executive officer. “From connecting older adults with nutrition programs to coordinating other essential supports, their work helps make it possible for older adults to remain safely at home and engaged in their communities. Nominations are now open for this year’s award. The 2026 Community Champion will be announced at the USAging Annual Conference and Tradeshow, July 18–21 in San Diego, Calif. Nominees do not need to be contracted with Mom’s Meals to be eligible; however, they must be from a USAging member agency. The deadline for nominations is Friday, May 1.
