How to Qualify for Home-Delivered Meals in Oregon

Here are some of the most common ways you may be eligible for home-delivered meals from Mom's Meals through government assistance or a health plan benefit.

Medicaid Members with Health Related Social Needs (HRSN)

Can you or someone you support answer "yes" to all of the following questions?

  • Enrolled in a Medicaid plan?
  • Living with a developmental or intellectual disability?
  • Living with a complex physical or behavioral health condition?
  • Experiencing a high-risk pregnancy, currently pregnant or postpartum?
  • Known to have experienced a history of interpersonal violence?
  • Someone with repeated visits to the emergency room or crisis services?
  • Experiencing food insecurity?

Medicaid Members, Older Americans Act (OAA) Recipients, and PACE Program Participants

We offer home-delivered and site-delivered meals to eligible clients, including:

  • Medicaid Waiver members
  • OAA program recipients
  • PACE program participants
  • New and expecting mothers
  • People who require assistance with grocery shopping and/or meal preparation

Contact your case manager or service coordinator to find out if you qualify.

Medicare Advantage (MA) Health Plan Members

If your MA health plan includes home-delivered meals as a covered benefit, you may qualify if you:

  • Have one or more chronic health conditions
  • Are a patient discharging from a hospital or skilled nursing facility or are at high risk of hospitalization

Check your health plan coverage manual to confirm if medically tailored meals are included and if you're eligible to receive them.

Complete a HRSN request form

For those enrolled in the Oregon Health Plan (Medicaid) and potentially eligible for Health-Related Social Needs (HRSN) services, visit the Oregon Health Authority (OHA) website to find your Coordinated Care Organization (CCO) and complete an HRSN request form.

What is Oregon’s Health-Related Social Needs (HRSN) program?

The HRSN program helps eligible Oregon Health Plan (OHP) members access non-medical services that support better health outcomes, including:

  • Medically tailored meals and nutrition services
  • Housing services
  • Climate supports

Delicious Meal Options

Who is eligible for meals?

To qualify, individuals must meet all of the following:

  • Be an Oregon Health Plan (Medicaid) member1
  • Be in an “HRSN eligible Population”.2 Examples are individuals who are or have been:
    • Discharged from an eligible behavioral health facility
    • Released from incarceration
    • In the Oregon child welfare system
    • Transitioning to dual eligible status
    • Homeless or are at risk of homelessness
    • Young adults with special health care needs (YSHCN)
  • Meet one of the clinical criteria.3 Examples are individuals who:
    • Have complex behavioral health or physical health needs
    • Need assistance with ADLs/IADLs or are eligible for LTSS
    • Are pregnant, experiencing a high-risk pregnancy or are postpartum
    • Have experienced interpersonal violence
    • Have repeated Emergency Department use and crisis encounters
  • Meet the USDA definition of low food security4

*Eligibility criteria may vary by health plan.
