Mom’s Meals, a leading provider of medically tailored, home-delivered meals, has announced its latest Community Champion: Lisa Zonneville, nutrition services coordinator for the Wayne County Department of Aging & Youth in Lyons, New York.
The Mom’s Meals Community Champion program honors case managers who raise awareness about nutrition and health in their local communities and positively impact the clients they serve. The program also seeks to promote the role of nutrition in managing chronic health conditions, living independently at home, and reducing the impact of social determinants of health. Case managers are nominated for the award by their peers or supervisors and do not need to be contracted with Mom’s Meals to be eligible.
In her role, Zonneville oversees five congregate meal sites across Wayne County in upstate New York as well as the provision of meals to seniors at home. People can socialize at the meal sites and learn about the importance of nutrition. In addition, the Wayne County program offers a take-out service to ensure that older individuals, including those with complex health conditions, receive nutritious meals.
“Case managers are true champions,” said John Phillips, vice president of national program development for Mom’s Meals. “As a direct result of their work, people with unique needs, many of them our most vulnerable neighbors, are receiving the services they require to manage and maintain their health and remain in their homes.”
Honie Allen, aging services assistant at the Wayne County Department of Aging & Youth, who nominated Zonneville, said, “Lisa strives to make sure our nutrition program runs flawlessly. She handles everything: the paperwork, the meetings and coordinating services.” For those who are homebound, Lisa makes sure they are able to receive meals.
Zonneville’s commitment to the local community goes beyond her immediate responsibilities. She distributes farmer’s markets coupons so that seniors with lower incomes can purchase fresh fruits and vegetables and help support local farmers. And she has collaborated with The Arc Wayne and the Wayne County Humane Society, so that people who receive home-delivered meals can also receive food for their pets through a donation-based program.
“At a time when so much is uncertain in our world, it’s comforting to know that people can count on their case managers in every sense,” Phillips also said.
