California Community Supports Inquiry Form

 

If you are a resident of California covered by Medi-Cal and feel you meet the guidelines for home-delivered meals under the Community Supports program, submit the inquiry form below and we will confirm your eligibility with your health plan.

Eligibility requirements to qualify may vary by health plan. Some plans may require a nutrition assessment to be completed to confirm eligibility for meals. The assessment will be completed by phone.

This request lets Mom’s Meals contact my doctor for information to help approve meals from my health plan. It also means the Medi-Cal member agrees to receive meals from Mom’s Meals if approved.
